After your policy is bound, these are the critical compliance steps we handle to keep your operation legal, insured, and on the road.

FMCSA Filings

Within 24 hours of binding
  • MCS-90 endorsement attached to the auto liability policy
  • Electronic filing via FMCSA's SAFER system
  • BMC-91X filing for freight brokers (if applicable)
  • Verification that filings appear on SAFER/FMCSA portal

DOT Number Verification

At binding
  • Confirm active USDOT number status
  • Verify operating authority (MC number) is active
  • Check biennial MCS-150 update is current
  • Validate USDOT number matches policy named insured

Certificate of Insurance (COI) Issuance

Within 24 hours of binding
  • Issue ACORD 25 (Certificate of Liability Insurance)
  • Include all required coverages and endorsements
  • Distribute to certificate holders (shippers, brokers, lessors)
  • Set up automated COI tracking and renewal reminders

Loss Payee & Additional Insured Setup

Within 48 hours of binding
  • Add lienholders as loss payees on physical damage coverage
  • Add lessors as additional insureds as required by lease agreements
  • Add shippers/brokers as additional insureds per contract requirements
  • Issue endorsements and updated COIs to all parties

Driver Qualification File Requirements

Ongoing — before any driver operates
  • Motor Vehicle Record (MVR) checks for all drivers
  • CDL verification — confirm valid Commercial Driver's License with proper endorsements
  • DOT medical certificate (Medical Examiner's Certificate) on file
  • Drug & alcohol testing compliance (DOT pre-employment and random)
  • Annual review of driving record per FMCSA §391.25

Quarterly / Annually
  • Annual premium audits — verify reported revenue, mileage, and units
  • Safety rating monitoring via FMCSA SMS (Safety Measurement System)
  • CSA (Compliance, Safety, Accountability) score tracking
  • Policy renewal preparation — loss runs, experience mod updates
  • Mid-term policy changes — adding/removing vehicles and drivers
